  3. If the therapy is provided by a company that is separate from the nursing home operator, report and concerns to the company providing the therapy.
  4. The nursing home is responsible for every agency and company they hire to provide care, so ask the nursing home for a list of which people and companies they have hired.
  5. Report the poor care, intentional bad acts, and neglect to the Illinois Department of Public Health. Be specific, detailed and complete.
